At Faith Home Care Practitioners PLLC , we are focused on providing medical care services to patients who are homebound or have difficulty getting in to see their primary care providers.
Our providers share a combined experience of over 20 years in family practice and gerontology. They are experienced in diagnosing, treating, prescribing, and managing acute and chronic care conditions, as well as providing primary care services with the highest level of customer satisfaction. With a variety of services to choose from, we're sure you'll be happy working with us.

Our Team
Julie Calderon,
DNP, MSN, ARNP, FNP-C
Dr. Julie Calderon is a family nurse practitioner licensed in Washington state, and has been in practice since 2013. She founded Faith Home Care Practitioners in 2015 when she observed how difficult it was for her older adult patients to access care due to their impairments.
She has a B.S. in Healthcare Administration from California State University, B.S. Nursing from Ferris State University, Masters in Nursing from Michigan State University, and a Doctorate in Nursing from Missouri State University.
She has a passion for chronic disease management and caring for the geriatric population. She has been a healthcare entrepreneur with her spouse, Emilio, since 2007. She is interested in developing and implementing new care delivery models that improve patient access and quality of care.
Outside of work, she enjoys spending time with her family, including her two teenagers, Olivia and Emilio Jr. She also enjoys traveling, binge watching movies and K-dramas, and listening to K-pop music with her daughter.
Cattleya Nery,
MSN, ARNP, AGPCNP-BC
Cattleya Nery is a compassionate, board certified Adult Gerontology Primary Care Nurse Practitioner dedicated to enhancing the quality of life for older adults.
Cattleya holds a Master of Science in Nursing with a specialization in Adult Gerontology Primary Care from Walden University and a Bachelor of Nursing from Ferris State University.
Her practice emphasizes holistic, patient-centered care, focusing on managing chronic conditions, preventing disease, and promoting healthy aging. Her approach is characterized by empathy, patience, and a genuine commitment to improving her patients' quality of life.
Outside of her professional duties, Cattleya enjoys running and staying active, Audible books, and spending time with her husband and 3 daughters.
Karla Rotante, RPH
Karla Rotante is a Clinical Pharmacist with seven years of professional experience, primarily serving as a Hospital Pharmacist. She earned her Bachelor of Science in Pharmacy, majoring in Clinical Pharmacy, from the University of the Immaculate Conception in Davao City, Philippines.
Throughout her career, Karla has remained committed to providing thoughtful and compassionate care to her patients. She strives to offer the support and assistance they need while continually looking for opportunities to strengthen her knowledge and improve the quality of her work. Her dedication to patient care and professional growth reflects her genuine desire to make a positive difference in the lives of those she serves.
Karla brings a caring and hardworking approach to her profession, and we are very excited to have her as part of our team. We look forward to seeing her continue to grow and contribute her skills, experience, and passion for patient care.
Outside of work, Karla is also a devoted full-time mother who enjoys bringing her daughter, Samantha, along wherever she goes. Family is an important part of her life, and when she isn’t busy with work or motherhood, there is apparently one more item on the agenda: her partner would love for her to cook pasta more often!
Evelyn Ferrer, RN
Evelyn Ferrer is a dedicated Registered Nurse with clinical experience in Medical-Surgical Nursing and a strong commitment to providing safe, compassionate, and patient-centered care. She is a graduate of Saint Michael’s College of Laguna in the Philippines, where she developed the knowledge and clinical foundation that prepared her for a career focused on caring for and advocating for others.
Throughout her nursing experience, Evelyn has developed a strong background in providing comprehensive patient care, collaborating with multidisciplinary healthcare teams, and supporting patients through various stages of treatment and recovery. She values clear communication, teamwork, attention to detail, and creating a supportive environment where patients feel heard, respected, and cared for.
Evelyn is passionate about patient advocacy and believes that quality nursing care goes beyond addressing a patient’s immediate clinical needs. She strives to approach each patient with kindness, patience, professionalism, and compassion while helping promote their comfort, safety, and overall well-being. Her dedication to excellence motivates her to continue strengthening her professional skills and expanding her nursing knowledge.
Outside of her professional life, Evelyn enjoys traveling, discovering new places, and spending time watching Korean dramas. These interests provide her with opportunities to relax and recharge while maintaining a healthy balance between her personal life and her passion for nursing.
Vanessa Rafallo, RN, BSN
Vanessa Rafallo is a dedicated healthcare professional from Quezon City, Philippines, with a strong foundation in patient care and clinical excellence. A licensed Registered Nurse with a Bachelor of Science in Nursing, she brings both knowledge and compassion to every aspect of her work.
Vanessa has extensive experience as an emergency room bedside nurse in a Joint Commission International–accredited hospital, where she learned to work efficiently under pressure while delivering high‑quality, patient‑centered care. Her time in the fast‑paced ER environment strengthened her clinical skills, critical thinking, and resilience in managing diverse and urgent medical situations.
In addition to her nursing background, Vanessa also served as a pharmacy assistant in one of the country’s leading pharmacy chains. This role deepened her understanding of medication management and further honed her attention to detail, teamwork, and customer service.
Known for her strong communication skills and warm, reassuring approach, Vanessa consistently builds trust with patients and colleagues alike. She takes pride in providing not only competent care but also comfort and emotional support to those she serves.
Beyond her professional life, Vanessa is a devoted mother to three beautiful children. She cherishes spending quality time with her family and finds joy in nurturing and supporting them. Recently, she has also developed an interest in joining fun runs as a way to de-stress, stay active, and maintain a healthy balance between her personal and professional life.
Anna Malveda
Anna serves as the Assistant to our Practice Manager, supporting the day-to-day administrative and operational needs of our practice.
She holds a Bachelor of Science in Business Administration, major in Export Management, as well as a degree in Culinary Arts. Anna previously owned and managed a clinical call center, where she gained valuable experience in healthcare operations, leadership, patient communication, scheduling, and administrative coordination.
In her current role, Anna assists with medical records, new patient registrations, provider orders, documentation, and other administrative responsibilities, working closely with our providers and staff to keep operations organized and efficient.
Outside of work, Anna is a devoted wife and mother of two. She enjoys spending time with her family and sharing her passion for cooking and bringing people together through good food.
Anna’s combination of business education, healthcare experience, leadership, and warm personality makes her a valued member of our team.
Emilio Calderon
Emilio Calderon has served as FHCP’s Practice Manager since 2015. He brings more than 20 years of customer service experience and 13 years of healthcare entrepreneurship to his role. Bilingual in English and Spanish, Emilio is passionate about helping improve healthcare delivery for patients with limitations.
Before joining FHCP, Emilio owned and operated a non-emergency medical transportation company that provided transportation for patients traveling to and from medical appointments. His experience has given him a strong appreciation for the challenges patients may face when accessing healthcare services.
Outside of work, Emilio is devoted to his family and an avid sports fan who enjoys cheering on the Seattle Seahawks, Los Angeles Lakers, Oregon Ducks, and Real Madrid. He is also an avid toy collector and proud sneakerhead.
